Perindopril for cardiovascular disease

What is Perindopril?

Over the past decade, the value of angiotensin-converting enzyme (ACE) inhibitors in treating hypertension has been well established, making them a first-line treatment. Perindopril is a non-sulfhydryl ACE inhibitor, chemically related to enalapril, and has stronger and longer-lasting ACE inhibition compared to enalapril. It is a potent and long-acting ACE inhibitor with benefits in protecting organs from hypertension and heart failure damage. Currently, it is primarily used for treating various forms of hypertension and congestive heart failure (CHF).

Perindopril uses

Perindopril has several indications as an ACE inhibitor, including primary hypertension, stable coronary artery disease (to reduce the risk of cardiac events in patients with a history of myocardial infarction and/or revascularization), symptomatic coronary artery disease or heart failure, and diabetic nephropathy.

How Does Perindopril Help in Treating Hypertension?

Perindopril is an ACE inhibitor that lowers blood pressure by reducing the formation of angiotensin II. It is a prodrug that inhibits ACE in humans and experimental animals. The mechanism of blood pressure reduction primarily involves inhibiting ACE, a peptidyl dipeptidase that catalyzes the conversion of the inactive decapeptide angiotensin I to the active vasoactive peptide angiotensin II. Angiotensin II is a potent vasoconstrictor that stimulates the adrenal cortex to secrete aldosterone and provides feedback to regulate renin secretion. Inhibition of ACE lowers plasma angiotensin II, reducing vasoconstriction, decreasing aldosterone secretion, and increasing renin secretion. The reduction in aldosterone leads to diuresis, sodium excretion, and hyperkalemia.

Can Perindopril Be Used to Treat Heart Failure?

Perindopril is a long-acting ACE inhibitor that exerts its effects through its active metabolite, perindoprilat. It works by inhibiting the conversion of angiotensin I to angiotensin II and the degradation of bradykinin, thus reducing the vasoconstriction and left ventricular remodeling associated with heart failure. In single-dose and long-term (8 weeks and 3 months) studies in patients with congestive heart failure (CHF), 4 mg of Perindopril significantly improved various hemodynamic parameters with minimal impact on blood pressure or heart rate. In a 3-month randomized, double-blind, placebo-controlled trial and a large non-comparative study (up to 30 months), daily administration of 4 mg Perindopril significantly improved exercise tolerance and reduced symptoms of heart failure in patients with mild to moderate CHF. Daily 4 mg doses of Perindopril are generally well tolerated in these patients. In a large non-comparative study, the most common adverse event was cough, leading to discontinuation in 2.8% of patients. In a short-term comparative trial, the incidence of initial dose hypotension was significantly lower with the recommended starting dose of 2 mg Perindopril compared to similar doses of captopril, enalapril, and lisinopril.

Is Perindopril Effective After a Heart Attack or Surgery?

Perindopril is effective in reducing the risk of future strokes and heart attacks. If used after a heart attack or heart surgery, it can also improve survival rates. While Perindopril can be used to prevent recurrent heart disease, not all other ACE inhibitors have received formal approval for such indications. Always follow your doctor's advice and the guidance provided in the medication instructions to ensure safe and effective use.

Is perindopril a blood thinner, yes or no?

Perindopril has antiplatelet properties, but does not negatively affect coagulation or fibrinolysis activities or interact adversely with anticoagulants. Therefore, it is of significant value in new strategies for hypertension treatment. As an ACE inhibitor, Perindopril effectively relaxes and dilates blood vessels, thereby lowering blood pressure and making it easier for the heart to pump blood throughout the body.

Dosage and Administration of Perindopril

Perindopril dose

The dosage of Perindopril varies depending on the specific condition being treated, so it is important to follow your doctor's instructions. To determine the most suitable dosage, your doctor will monitor your blood pressure and ask about any side effects. Blood tests may also be performed to assess kidney function and potassium levels. Perindopril can be found in various forms, including Perindopril, erbumine (also known as Perindopril tert-butylamine), and Perindopril arginine, though their dosing may differ despite similar mechanisms of action.

Perindopril (Coversyl in the UK, Aceon in the US; for treating hypertension, 4 to 8 mg once daily) is long-acting with a favorable peak-to-trough ratio. International brands of Perindopril erbumine (Aceon) include:

  • Acertil (Mainland China, Hong Kong, South Korea, Taiwan)
  • Coverene (Argentina)
  • Coversum (Austria, Germany, Switzerland)
  • Coversyl (Belgium, Brazil, Canada, Chile, Colombia, Costa Rica, Denmark, Dominican Republic, Ecuador, El Salvador, UK, France, Greece, Guatemala, Honduras, India, Ireland, Italy, Japan, Kuwait, Malaysia, Mexico, Netherlands, Nicaragua, Panama, Paraguay, Philippines, Portugal, South Africa, Spain, Thailand, UAE, Venezuela)
  • Perinace (Malaysia)
  • Prexum (Indonesia)

When to take perindopril?

Typically, Perindopril is recommended to be taken once daily, swallowing the tablet whole with water. Some may wonder Why is perindopril taken at night? Doctors might advise taking the first dose before bedtime to avoid potential dizziness. If no dizziness is experienced after the initial dose, you might consider taking it in the morning, preferably 30 to 60 minutes before breakfast. Always adhere to your doctor's guidance and the instructions provided with the medication.

Warnings

Perindopril has demonstrated significant efficacy in controlling hypertension, improving heart failure, and preventing cardiovascular events. However, as each patient's health condition and needs differ, it is crucial to consult a doctor or healthcare professional when using Perindopril. Professional medical advice will ensure the safe and effective use of this medication, achieving the best therapeutic outcomes.
